Who can play? All students in grades 5-8 Requirements All students and their parents must complete the following items before receiving a uniform jersey and shorts 1. Sport sign up form 2. Waiver of Liability 3. Athletes Contract 4. Parental Consent Form 5. Payment of fee and return uniform Medical Forms No student may practice unless he or she turns in a Catholic Schools Health Form for the Diocese of Ft.
